Request an accreditation review

This form collects the information needed for an initial review of an accreditation request. It does not create an accreditation contract, guarantee acceptance or constitute an accreditation decision.

Before submitting

IAQCA reviews the identity of the applicant, the requested scope, possible conflicts of interest, competence needs and available resources. The request is assessed before any formal proposal or evaluation arrangement is issued.

What happens after the request

Initial review

IAQCA checks identity, scope, completeness and possible impartiality risks.

Clarification

The applicant may receive questions or a request for additional information.

Formal proposal

Where the request is eligible and resources are available, IAQCA communicates the applicable pathway and conditions.

Independent decision

Any accreditation decision is made independently from the evaluation team and is published only with controlled status information.

Scope limitation

IAQCA Accreditation does not directly certify persons. This request concerns accreditation activities within the published IAQCA scope.
